Medical / Release Form

This a medical release form, much like a school physical form, a release to participate, and a publicity release. The medical part must be completed by a doctor and the form must be updated every three years.

Keep a copy of your medical / release form in case it is lost in the mail and so you can check the expiration date later.

We recommend completing a new form at every annual doctor's visit.

Unified Partners Form

This form is for people who play with our athletes on unified teams.

Volunteer Form

This form is for all volunteers, youth and adult.
